Participant scoring form data helps us in our continual
efforts to improve the reliability and validity of our
products. So when you send us your participant data we’ll

of outstanding college graduates teaching urban and rural
public school students while becoming lifelong leaders in
education equity and excellence for all children. Since

It’s easy, too. Simply collect participant scoring forms at
the conclusion of your training event and return them to us
at the address below or use the pre-addressed envelope
enclosed with your order. The more forms you send, the more
we contribute!

Privacy
Please know that we value your privacy. Personally
identifying information from your participants and
information about your organization will not be stored with
scoring data. Any personal information will be treated
confidentially. Anonymous data is always welcome.
Drawing Rules
Drawings are held every three months. Winners will be
notified by phone or mail. Names remain in the drawing pool
unless they are drawn as winners. If you are drawn as a
winner, you may re-enter the drawing upon receipt of
additional data. Let us know if you would prefer that we not
register you for the drawing. In order to be registered to
win, you consent to limited, reasonable use of your company
name in a list of contributing organizations on our website.
HRDQ may end the drawing at any time.
Instructions:
1.
During your training event, ask your participants to
write their gender, age, title, and industry on the last
page of the scoring form. Demographic information helps
us refine our analysis and is treated confidentially.
Information identifying individual participants or
organizations is not retained by HRDQ. Anonymous data is
also accepted.
2. Gather the last page of the form from participants at
the conclusion of your training event. Photocopies are
also acceptable.
3. Place the scoring forms in the self-addressed
envelope included with your assessment order or use any
envelope. Include your business card so we can enter you
in our drawing for a $100 American Express gift card.
